SAWI Member and Returning Attendee Discount!
Members of the Society for the Advancement of Women’s Imaging (SAWI) and attendees of the 2005, 2006 or 2007 Chicago International Breast Course can deduct $100 from their course registration fee. Sorry, no discount on the daily fee! For details of registration fees, please see registration form.

On your registration page/form you will need to indicate workshop choices. You can only register for ONE SESSION per day. Registration is limited and is strictly on a first come/first served basis.

You will need to indicate whether you would like to receive your syllabus on a CD OR printed and 3-ring bound. You can receive both the CD and a 3-ring bound syllabus for a fee of $15.

- By mail: Please fill out our paper registration form that is attached to the brochure. Please make checks payable to: Northwestern Radiology. We only accept checks drawn on a U.S. bank account and payable in U.S. funds.
Refund Cancellation Policy
The registration fee less a $50 handling fee is refundable if a written request is received on or before August 15, 2008. No refunds will be granted after August 15, 2008, however, you may transfer your registration to a colleague.
Accomodations
The Westin Chicago River North, located at 320 North Dearborn, is located on the scenic bank of the Chicago River and is conveniently located in the heart of Chicago’s business, shopping, and theater districts.

Ground Transportation
Chicago is served by two airports: O’Hare International and Midway. Taxicabs are available at an estimated cost of $45-50 to or from O’Hare and $30-35 to or from Midway. In addition, there is an Airport Express shuttle that runs directly to the Westin River North Hotel from both airports. Arrangements for the shuttle can be made in the Baggage Claim area at either airport or in advance at www.airportexpress.com. From O’Hare, the one-way rate is $27 and $49 round trip. From Midway, the one-way rate is $22 and $37 round trip.
